Health Records

View and manage patient health records, including medical history, medications, and ICD codes.

The Health Records tab on a patient's detail page shows comprehensive clinical information and health data. Meelio enriches this data to provide better context for clinical reasoning.

What's Included

Health records are organized into several categories:

Medical History & Conditions

  • Conditions — Chronic and acute medical conditions
  • ICD Codes — Many conditions are enriched with standard ICD-10 or ICD-11 codes for better interoperability and AI reasoning
  • Status — Active or inactive status for each condition

Medications & Supplements

  • Medications — Current and past prescriptions
  • Details — Dosage, frequency, and duration (when available)
  • Instructions — Expandable sections for specific patient instructions and administration details

Allergies & Sensitivities

  • Known allergies and their reaction severity

Lab Results & Vitals

  • Recent lab values, test results, and vital signs (e.g., blood pressure, weight, BMI) formatted for easy review

Data Sources

Health records come from two primary sources:

EHR Integration

If you've connected Cerbo or Practice Better, patient health data is synced automatically. The sync pulls in demographics, medical history, medications, and other clinical data. Meelio automatically deduplicates information when syncing from multiple sources.

Meelio-Generated Records

Clinical notes, care plans, and other documents generated through Meelio consultations are also saved to the patient's record and can be viewed in their respective tabs.

Using Health Records with AI

When you start a consultation or AI chat for a patient, Meelio provides the AI with the patient's full health record as context. This enables:

  • Condition-aware documentation — Clinical notes that reference existing diagnoses
  • Safety checks — Identifying potential contraindications in care plans
  • Longitudinal tracking — Reasoning about how health trends have changed over time
